Morphofunctional rehabilitation of the congenital hand with fingers absence or deficiencies involves an early reconstruction of the digital pinch using tissues similar in function and aspect. In our opinion the foot fingers transfer represents the best method. Our casualty consist of 8 cases operated in the last two year with absence or malformation of one digital ray – 3 cases, II-V digital rays malformation – 5 cases. The age of the patient varied between 8 to 22 years and they were predominately girls - 6 cases. Our method consists in a free digital transfer of the great toe – 1 case, second toe - entirely (3 cases) partial (1 case), and digital block of the second and third toe – 3 cases. The evolution was favorable in all cases. The mobility was evaluated from M2 to M4 and the sensibility from S2 to S3 at one-year postoperative. Digital transfer represents the best therapeutic option in our opinion. The intervention must be performed as early as possible considering the better cortical integration in the young patients. Even if the method is technically difficult the results are better with a shorter recovery and hospitalisation.
