Case 1: A 18 year old male sustaining of severe car crash accident , involving the lower third of leg under went several operation such as debridment , local flaps , gastrocnomious transfer, all failed … The Patient remained with exposed infected bone along with 15*12 cm soft tissue loss. The foot was alive and sensed with only tibialis posterior artery and nerve with scarred, not to take the risk of thrombosis of the zone of injury .After proper debridment , excision of the involved and dead bone stabilizing to leg together with orthopedic devices , latissmus dorsi myocotaneous free flap was transferred , anastomosis were performed to the contra lateral dorsalis pedis and muscle covered the defect .After 5 weeks the pedicle of the muscle was divided and the flap survived well. Case 2 : A 50 year old man sustaining a car accident with severe crash of the lower third of the leg with head injury who was in coma for around 2 months with left facial nerve lesion. The patient went also under several procedures such as local flaps reversed gastrocnomious transfer …… which all failed. So the same procedure was applied and the same result was obtained. In both cases rubber band-test and intermittent delay started ;after weeks from 5 min. squeezing and one hour release , to one hour squeeze and one hour release. The trial and conclusion will be discussed along with slides in lecture.
