Lunotriquetral Arthrodesis: Good Results and Low Morbidity

As suggested at the AASH annual meeting 2002, treatment for lunotriquetral interosseous ligament (LTIL) injury remains controversial. Seven consecutive patients underwent lunotriquetral (LT) arthrodeses utilizing a headless cannulated compression screw and compressed autologous bone graft. All patients were arthroscopically diagnosed with stage 3 or 4 (Geissler/Freeland) LTIL tears. Two patients had isolated LT tears, and five had more extensive perilunate injury addressed.

All patients exhibited radiographic union in an average 8.5 weeks. No nonunions were seen. Five patients report good relief of symptoms associated with LTIL arthrodesis. One patient reports fair pain relief and is satisfied with the results. One patient underwent partial wrist denervation and subsequently reports good pain relief. There were no major complications.

We report our surgical technique and rehabilitation program which has provided high (100%) union rate with low risk for iatrogenic nerve injury.
